چکیده

The aim of this chapter is to evaluate the contribution of antimicrobial therapeutic drug monitoring (TDM) to adjust dosing regimens in achieving control of infections using pharmacokinetics (PK) and pharmacodynamics (PD) to define the antimicrobial exposures that are necessary to maximise bactericidal activity and to inhibit bacterial growth. Robustness data exist for some antimicrobial agents, including the ratios of PK/PD parameters to the minimal inhibitory concentrations of bacteria that are associated with maximal antimicrobial effects (e.g., peak drug plasma concentration, Cmax:MIC ratio; area under the curve, AUC0-24:MIC ratio; and time interval required to maintain the minimum plasma drug concentration above the minimum inhibitory concentration, ΔT>MIC). The application of PK/PD tools for the effective control of severe infections must be considered for systemically administered antimicrobial agents that are prescribed to burn patients and other critically ill patients in the intensive care unit.

Pharmacokinetic/Pharmacodynamic (PK/PD) Relationship-based Approach to Understand the Variability of Central Neurological Effects of Ethanol

Pharmacokinetic/pharmacodynamic (PK/PD) relationships describe the quantitative relationships between the drug-induced effects and the concurrent corresponding drug concentrations in an individual. Since a long time, rough qualitative relationships have been described between neurological presentation and plasma ethanol concentration in acute ethanol ingestion. However, to date, precise quantit...

Antimicrobial tissue concentrations.

The clinical outcome of anti-infective treatment is determined by both PK and PD properties of the antibiotic. Only the free tissue concentrations of antibiotics at the target site, which are usually lower than the total plasma concentrations, are responsible for therapeutic effect. The free antibiotic concentrations at the site of action are a more appropriate PK input value for PK-PD analysis...

An international, multicentre survey of β-lactam antibiotic therapeutic drug monitoring practice in intensive care units.

OBJECTIVES Emerging evidence supports the use of therapeutic drug monitoring (TDM) of β-lactams for intensive care unit (ICU) patients to optimize drug exposure, although limited detail is available on how sites run this service in practice. This multicentre survey study was performed to describe the various approaches used for β-lactam TDM in ICUs. METHODS A questionnaire survey was develope...

How severe is antibiotic pharmacokinetic variability in critically ill patients and what can be done about it?

The pharmacokinetics (PK) of antimicrobial agents administered to critically ill patients exhibit marked variability. This variability results from pathophysiological changes that occur in critically ill patients. Changes in volume of distribution, clearance, and tissue penetration all affect the drug concentrations at the site of infection. PK-pharmacodynamic indices (fCmax:MIC; AUC0-24:MIC; f...

Pharmacokinetic/pharmacodynamic (PK/PD) considerations in the management of Gram-positive bacteraemia.

Bloodstream infections are amongst the most serious infections of hospitalised patients and are associated with high mortality, especially amongst those with severe sepsis and septic shock. A range of organ dysfunctions, together with drug interactions and other therapeutic interventions (e.g. haemodynamically active drugs and continuous renal replacement therapies) may have a strong impact on ...
